  • Compounding Pharmacist Dr. Matt Rease: Helping Alpha-gal Syndrome Patients

    October is American Pharmacists Month, a time to recognize all pharmacists’ efforts to care for their communities. We had the honor of speaking with Dr. Matt Rease, PharmD, the pharmacist-in-charge at Christiansburg Pharmacy in Christiansburg, Virginia. Their independent pharmacy offers custom compounding services to help alpha-gal patients gain access to animal-free medication. When did you first learn about alpha-gal? I first encountered the term Alpha-Gal through one of my patients at the Pharmacy a few years ago. The other pharmacists and I were not familiar with the syndrome, and this led to us to begin researching the subject to find ways to help these patients. Your pharmacy compounds animal-free medication for patients with alpha-gal syndrome. For those who are interested, how can they request this service? Compounding medication is a great option for Alpha-Gal patients who cannot find a suitable product commercially available. Depending on the medication, the pharmacy can research to see if the active ingredient is available in bulk powder to compound. We are able to compound capsules using vegetable derived shells or liquid/suspension-based medications. We can compound with no additives and use food-based products as filler (e.g., rice flour) when needed. Is this for anyone in Virginia? At this time, we only fill prescriptions for our local patients as we are not licensed as a mail-order pharmacy. Patients interested in compounding services should locate a local pharmacy near them that has the capability of accommodating these needs. Can you share a successful patient story with us? It’s always special to provide a patient with a solution. I remember one customer in particular who had searched seemingly everywhere for answers on how to obtain one of their medications in a specific composition. We found that we could compound the medication using vegetable-based capsules under the cleanliness of a laboratory hood, thus safely providing them with the medication they required. What is your favorite part of your job? As a local independent pharmacy, we are passionate about providing excellent customer service. I personally enjoy providing my patients with solutions to their problems, many of which were left unanswered at other pharmacies. What advice do you have for patients with any type of allergy trying to find medication or supplements? I would advise them to find a pharmacy that will take the time to invest extra work, and provide them with the information they seek for their healthcare needs. Is there anything else you would like to share with us? I’m often surprised by the lack of transparency that many manufacturers provide regarding what is in their products. Many times, they cannot provide the answer as to whether mammalian byproducts are in their products. I think there should be more accountability for these manufacturers, and more awareness raised about the significant needs of individuals like those with alpha-gal syndrome.     As we often hear about the importance of essential vitamins and minerals, one vital nutrient that many people talk about is Vitamin D3, commonly known as the "sunshine vitamin." Vitamin D3 ( cholecalciferol) plays a crucial role in bone health, muscle function, and overall well-being. Vitamin D3 Requirements To prevent vitamin D deficiency and ensure adequate intake, it's crucial to maintain a sufficient level of Vitamin D3. The table above shows the Recommended Daily Intake of Vitamin D3 based on data from the NIH (National Institutes of Health) Office of Dietary Supplements and Dietary Guidelines for Americans. The maximum daily intake for vitamin D in adults and children ages 9+ is around 4,000 IU (100 mcg). Studies suggest that long-term supplementation with vitamin D3 in doses ranging from 5,000 (125 mcg) to 50,000 (1,250 mcg) IUs/day appears to be safe. Causes of Vitamin D Deficiency Vitamin D deficiency is a common condition that can affect people of all ages. It can occur due to a combination of various factors including: Inadequate sun exposure : The body can produce vitamin D when the skin is exposed to sunlight. Age and obesity : Older adults and people who are obese are more likely to experience vitamin D deficiency. Geographic location : People who live in northern latitudes, where there is less sunlight during the winter, are more likely to be vitamin D deficient. Malabsorption disorders (e.g. Crohn's, Celiac disease): People with malabsorption disorders are at risk of vitamin D deficiency because they cannot absorb nutrients as well. Lack of dietary sources : People who do not consume enough foods that are rich in vitamin D are more likely to be deficient. Medications : Some medications, such as anticonvulsants and weight loss drugs, can contribute to vitamin D deficiency. Symptoms of Vitamin D Deficiency Vitamin D deficiency can lead to a range of symptoms, which can vary depending on the severity of the deficiency. Common symptoms include: Bone and muscle pain : Vitamin D is essential for the absorption of calcium and phosphorus, which are important for bone health. Vitamin D deficiency can lead to bone pain, muscle weakness, and muscle cramps. Fatigue and weakness : Vitamin D is involved in energy production. Vitamin D deficiency can lead to fatigue and weakness. Decreased immune function : Vitamin D helps the body fight off infection. Vitamin D deficiency can lead to a decreased immune function, making people more susceptible to infections. Mood and mental health : Vitamin D is involved in the production of serotonin, a neurotransmitter that regulates mood. Vitamin D deficiency has been linked to depression, anxiety, and other mental health problems. Cognitive effects : Vitamin D is involved in brain development and function. Vitamin D deficiency has been linked to cognitive decline, such as dementia and Alzheimer's disease. Vegan-Friendly Vitamin D3 Options Vitamin D3 supplements are available in various forms, including capsules, tablets, and gummies. When selecting a vitamin D3 supplement, consider a form that suits your lifestyle and is easy for you to consume.     We had the pleasure of interviewing Dr. John Burd, the founder and CEO of Lysulin, Inc .   • The Ugly Truth Behind Alpha-gal Syndrome: A Personal Story

    What is alpha-gal syndrome and why is everyone talking about it? Despite being known for years, alpha-gal syndrome (AGS) has recently garnered significant attention, thanks to the CDC and various media outlets. Also referred to as alpha-gal allergy, red meat allergy, and tick bite meat allergy, AGS is a serious condition caused by an allergy to the alpha-gal (galactose-α-1,3-galactose) sugar molecule. This condition is often linked to tick bites, particularly those of the lone star tick found in the United States. Individuals afflicted with AGS can experience serious allergic reactions upon consuming red meat or other products containing alpha-gal. Given the substantial amount of people affected by AGS, it demands greater attention. However, media coverage only scratches the surface, leaving many unaware of the hardships and emotional toll associated with this allergic condition. We aim to provide deeper insight into AGS, sharing with you a real-life story to showcase the profound impact it can have. Our team interviewed H.V., an AGS patient from a small rural town in Missouri. Through her personal story, we get to witness the challenges she confronts while living with this condition. H.V. has been surrounded and bitten by ticks countless times throughout her life but never had an issue until shortly after her 30th birthday. Her description of her hometown paints a picture of abundant wildlife, woodlands, national forests, and farms, rendering ticks unavoidable. She recalls instances where ticks even fell into her hair while walking under trees! In this interview, H.V. shares her long and tiring journey toward obtaining an accurate diagnosis and sheds light on the painful reality of managing this life-long allergic condition. Can you share your journey on how you were diagnosed with alpha-gal syndrome? “Shortly after my 30th birthday in 2003, my health began to take a dive. Within a few short years, I had Endometriosis, COPD/emphysema, Fibromyalgia, and Raynaud's Syndrome pinned to my medical chart. If all of that wasn't enough, along came a head-to-toe itch out of the blue. No explanation. My doctor told me to take Zyrtec and I have been taking it every day ever since. I can't go more than 24 hours without taking it or the intense all-over itch comes back. But this itch was just the beginning. As time went on I began to notice weird and unexplained things happening. I started reacting to dairy, specifically milk and ice cream, with excruciating stomach aches. Anything that contained jalapenos gave me migraines. I started getting terrible stomach aches from ibuprofen. Some prescriptions caused side effects that weren't even listed as possible side effects. And my list of prescription allergies began to grow. Fast forward to Nov. 2017. I got 2 vaccines and, within 2 weeks, I began reacting to everything! Everything I ate, every prescription I took (including an inhaler), even plain water. I had reactions on and off all day every day. I felt like I couldn't breathe. I wasn't sleeping well. Every time I dozed off, I woke up. My stomach was bloated and hurt constantly. Strong smells like perfume, candles, cleaners, and even gasoline gave me headaches and made me feel ill. I was exhausted. I had constant inflammation and pain throughout my entire body. I was a mess! In 2018, I spent most of the year bouncing from one doctor to another. My diet consisted of maybe 10 different foods. I ate the same thing for dinner night after night (salmon, rice, and a veggie). I went to the ER several times. They treated me like I was crazy and dismissed it as either anxiety and panic attacks or COPD related. I tried to explain that when you feel like you can't breathe, you're going to have anxiety and panic attacks. But that's not how they saw it. I was also not convinced it was COPD related. I saw several specialists, but I still didn't feel like the issue at hand had been found. After a sleep study, I was diagnosed with obstructive sleep apnea. The CPAP helped some with sleep, but I was still waking up in the middle of the night. I had symptoms of a heart attack in May 2018, which included elevated troponin levels. I spent 3 days in the hospital. They found nothing wrong with my heart after doing all of their testing. They gave me shots in my stomach to prevent blood clots. I had a large bruise on my stomach for a month. (I might add that I bruise very easily now and bruises are visible for weeks, sometimes months.) I had allergy testing done and tested positive for several foods (eggs, milk, peanuts, wheat, cashews, beef, chicken, pork, buckwheat, carrots, and tomatoes). I avoided those foods and still reacted to other foods. I went back to the allergist to get more testing done and was told it might just be intolerance to some foods and they refused to do any more testing. In Oct. 2019, my primary care NP at that time, who just happened to have Alpha Gal Syndrome herself, tested me for it. I had no idea what Alpha Gal Syndrome was, but it came back positive. I was told to avoid mammal meat, dairy, and by-products and take a vegan Vitamin D. Sounds easy enough. But it's definitely not been that easy. I have had to research and learn everything about Alpha Gal Syndrome on my own. It has been a very long and painful journey. No one should have to suffer like I have because it is simply inhumane.” What are the challenges that you’re facing with AGS? How has it affected your daily life? H.V. reveals that AGS has profoundly impacted every aspect of her daily life, including financial strain, social interactions, mental well-being, physical health, and even her intimate life. Unfortunately, many treatments and specialists crucial to her condition are not covered by her insurance. The cost disparity between vegan foods, vitamins, supplements, personal care items, and cleaning products in comparison to their non-vegan alternatives is striking, often amounting to 2-3 times the expense. In terms of her dietary choices, H.V. is afraid to eat anything that she hasn’t prepared herself. She has stopped dining out altogether and has to bring her own food to family gatherings. H.V. expressed that having AGS is a challenging reality to accept, given its lack of a definitive cure. Her doctors made her feel crazy and hopeless, which led her to find comfort in alcohol. Although her alcohol use has declined, she still goes to counseling regularly to help with her mental health. The constant need to meticulously scrutinize product labels also cause her to feel mentally exhausted. In addition to H.V.'s mental health, her physical health has also taken a toll. The array of food restrictions has led to deficiencies in vital nutrients, necessitating the use of supplements. She constantly has to visit her doctor for different issues, and certain labs, such as vitamin B12, vitamin D, and cholesterol levels have to be obtained periodically. Despite how careful she is, she still experiences reactions, such as cough, phlegm, stomach pains, and even waking up in the middle of the night with difficulty breathing. This highlights the necessity for healthcare professionals to be mindful of these modifications while prescribing treatments. 50% of individuals with AGS have experienced anaphylactic reactions to health products containing alpha-gal. This emphasizes the urgent need for improved awareness and preventive measures in both healthcare settings and patient education. 75% of AGS patients have had to reach out directly to drug manufacturers. This demonstrates the importance of fostering open lines of communication between patients, healthcare professionals, and the pharmaceutical industry to address the unique challenges faced by this patient group effectively. The conference theme, "ILLUMINATE," focused on diversity, innovation, and AI. Our research perfectly aligned with this theme, calling on healthcare professionals and the pharmaceutical industry to recognize and address the unique struggles faced by AGS patients. My husband and I have worked in conservation most of our lives, so we have always been aware of the urgency of addressing climate disruption. Only after I developed alpha-gal syndrome and became an advocate for the alpha-gal community did I realize how ubiquitous animal-derived ingredients are in drugs and other medical products and of the threats they pose to the alpha-gal community. Can you share a bit about what you do? I have worked in conservation in one capacity or another most of my life, but now my work as director of the Alpha-gal Syndrome Awareness Campaign and managing the Alpha-gal Information website take up a lot of my time.     Why is Iron Important? Without consuming red meats, fish, and poultry which are rich in iron, iron deficiency can be a concern for those leading a vegan or vegetarian lifestyle. About 70% of our body's iron is found transporting oxygen as hemoglobin and myoglobin in red blood cells and muscle cells respectively. With more than 3 million cases of iron-deficiency anemia reported in the US each year, following the “Fuel for the Future” theme for National Nutrition Month 2023, we can be proactive about our diet. Recommended Daily Iron Intake The National Institutes of Health's recommended dietary allowance (RDA) or daily intake of iron varies based on age and sex. For males 19 years of age or older, the recommended daily iron intake is 8 mg. For females 19 to 50 years of age, the recommended daily iron intake is 18 mg. Menstrual bleeding and pregnancy increase the need for iron while periods of lactation where menstruation is paused for the first few months after pregnancy decrease the need. Iron can be found in its heme or nonheme state depending on the food from which it is derived. Heme iron, a natural substance in the body that helps bind oxygen to red blood cells as hemoglobin, is found in animal meats such as beef, pork, chicken, and seafood. Non-heme iron is plant-derived and can be found in foods such as fortified breakfast cereals, white beans, lentils, spinach, tofu, and nuts. Signs and Symptoms of Iron Deficiency Iron deficiency can result in anemia as the body lacks the iron to produce enough hemoglobin-rich red blood cells. High-risk groups of iron deficiency anemia include women, the elderly, those on blood thinners, those with kidney failure, and those with difficulties taking in iron. If experiencing any signs and symptoms listed above and believe it may be due to iron deficiency, speak with your doctor as iron levels can be determined through blood tests.
