  • Putting Patients First: The Complicated Truth about Medications and Animal-Derived Ingredients

    Based on a true story. Names of persons, companies, and products have been kept anonymous to protect privacy. Patient X has alpha-gal allergy. They knew even a tiny amount of meat or mammalian-derived ingredients could trigger a reaction that could be life-threatening. That’s why when their doctor wanted to prescribe a new medication, they reached out to the Pill Clarity team first to figure out which medications are alpha-gal safe. Our team contacted the pharmaceutical company and explained the patient’s concern. They investigated and verbally told us that the medication did not contain any animal-derived ingredients and would send us a written confirmation. We passed the information to Patient X and they were relieved and grateful for the news. The following day, the pharmaceutical company sent an email confirming the conversation we had and provided additional information. However, when our pharmacists reviewed the document, our team discovered conflicting statements that required further clarification. We had to contact the company again to ensure that we had an accurate understanding of the situation. Despite the fact that the first sentence of the document stated that no animal-derived ingredients were used directly in the final formulation of the medication, our team discovered through further review and a second conversation with the pharmaceutical company that this was not entirely accurate. The document went on to reveal that certain ingredients used in the manufacturing process were sourced from animals, ultimately leading us to conclude that the medication was not safe for alpha-gal patients and was not free of animal-derived ingredients (see images below). Statement on the first page of letter received Statements found on the second page indicating presence of animal-derived ingredients Overall, Patient X along with our team were disappointed and frustrated. Fortunately in this instance, we prevented a possible reaction based on the patient’s previous history. Of note, Patient X appreciated our team for being honest and our diligence in securing accurate information. Transparency and accuracy in labeling is crucial for medications. It is estimated that over 1 million Americans suffer from alpha-gal syndrome (AGS) and 76% of them have previously reacted to animal-derived ingredients in medications. Further, 49% of those AGS patients reported having had an anaphylactic reaction to health products. Research has indicated that pharmaceutical companies are not always informed or understand the AGS or vegan patients. Some data suggests that the standard responses from a medical information center can be inaccurate 50% of the time. Due to the grave consequences for patients like AGS, it is imperative that independent and third-party certification processes are put in place to ensure that the information provided is trustworthy and accurate. The pharmaceutical industry must take this issue seriously and work towards providing clear and precise information to both healthcare professionals and patients. Thousands of people have benefited greatly from learning how to make healthier, more sustainable choices, and we plan to reach even greater numbers this year.” A Brief History of Black Veganism Despite the growing popularity of veganism in the media, the rich history of Black veganism is often overlooked. This history is closely tied to Black activism in the 1960s and the African tradition of plant-based diets. Spiritual movements like Rastafarianism and the Nation of Islam have encouraged plant-based diets, while civil rights leaders such as Angela Davis and Coretta Scott-King have promoted it.   • Treatment of High Cholesterol and Triglycerides with Animal-Free Statins

    What is Hypercholesterolemia and Hypertriglyceridemia? Hypercholesterolemia is defined as high cholesterol levels and hypertriglyceridemia is defined as high triglyceride levels. According to the Center for Disease Control and Prevention (CDC), approximately 1 in 3 adults had high cholesterol from 2015 to 2018, 1 in 4 adults had high triglyceride levels from 2009 to 2012, and heart disease was the number one cause of death in the United States 2020. High cholesterol and triglyceride levels can result in fatty deposits in blood vessels and vascular wall thickening and hardening. If left unchecked, this can lead to heart disease and fatal outcomes such as heart attacks. A common medication class used to lower bad cholesterol (LDL, TC) and triglycerides (TG) while increasing good cholesterol (HDL) is HMG-CoA Reductase Inhibitors, also known as Statins. Over-the-counter (OTC) red yeast rice, similar to Lovastatin, can also be used as a supplement for select patients. What are Animal-Free High Cholesterol Treatment Options? Statins are first-line medications in combating high cholesterol levels and are generally safe and effective, but many formulations we have researched are not animal-free. Statins often include animal-derived or potentially animal-derived ingredients like lactose, magnesium stearate, or polysorbate 80. However, our team is pleased to announce there are animal-free alternatives for Atorvastatin and Fluvastatin ( contact our medical information team for more information) ! Although statins are a prescription-only treatment option, there is an over-the-counter option in the yeast derivative of Monascus Purpureus, known as Red Yeast Rice . The monacolin K in red yeast rice is chemically identical to the active ingredient in Lovastatin, thus reducing low-density lipoprotein cholesterol, triglyceride, and total blood cholesterol levels while improving high-density lipoprotein cholesterol levels. However, the potential for an inconsistent amount of monacolin K from capsule to capsule of red yeast rice is a shortcoming to consider prior to consistent use and why red yeast rice supplementation is not recommended as a substitute for cholesterol-reducing medications. Side Effects of Statins and Red Yeast Rice Common side effects include: Heartburn Headache Potential abdominal discomfort Muscle aches Precautions with Statins and Red Yeast Rice Red yeast rice and statins are not recommended during pregnancy and while breastfeeding. According to the U.S. Food and Drug Administration (FDA), statins may cause fetal harm in those who are pregnant. In those taking statins: Red yeast rice carries Monacolin K which is chemically identical to Lovastatin which may increase the risk of adverse events. In those with current liver damage: Red yeast rice and Statins can increase the levels of liver enzymes and risk of hepatotoxicity. What Interacts with Statins and Red Yeast Rice? Products to avoid or use with caution when taking statins or supplementing with red yeast rice are: Alcohol Grapefruit juice St. John's Wort Cyclosporine Other Lipid Lowering Medications (Ex. Niacin, Gemfibrozil, Other Statins) Conclusion Although many statin medications contain animal-derived ingredients, we do not discourage the use of potentially life-saving medications which may not be animal-free and it is important to involve your doctor before making any changes to your health care regimen. The adoption of a vegan or vegetarian diet can help lower cholesterol, the risk of heart attack, and worsening coronary artery disease.   • What is Paxlovid and is it Vegan-Friendly?

    Paxlovid is a new oral medication that has been authorized for emergency use by the FDA in mild to moderate coronavirus disease 2019 (COVID-19) in adults and pediatric patients (12 years of age and older weighing at least 40 kg) with positive results of direct severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral testing, and who are at high risk for progression to severe COVID-19, including hospitalization or death. Paxlovid is the brand name for a combination of two separate active medications called Nirmatrelvir and Ritonavir. President Biden’s Test-to-Treat Initiative Through this newly launched initiative, patients who test positive for COVID-19 will be able to have quick access to potentially life-saving, Paxlovid. Patients will be able to go through local pharmacy-based clinics, federally qualified community health centers, and residents of long-term care facilities will be assessed by a health care provider (HCP) who has the authority to prescribe the medication. Who will need to take this medication? Who is high-risk for severe COVID-19 Infection? The CDC has a resource that details conditions that may put patients at high-risk for developing severe COVID-19. These include the following: What is in Paxlovid? Paxlovid is an antiviral medication that helps fight the COVID-19 virus. It is an inhibitor of the enzyme responsible for viral replication of SARS-CoV-2. It is comprised of two 150 mg tablets of Nirmatrelvir with one 100 mg tablet of Ritonavir, with all three tablets taken together twice daily for 5 days. The medication is recommended to be administered as soon as COVID-19 symptoms begin or within 5 days of symptom onset. Is Paxlovid Vegan? The short answer is no. Technically, medications are not considered vegan because they often require animal testing during the drug development process. The accurate question is, “Is Paxlovid animal-free?”. Paxlovid is not animal-free due to lactose monohydrate, an inactive ingredient within the Nirmatrelvir tablets, which is commonly associated with dairy products. What does the data show? Efficacy: A landmark randomized-control trial published in the New England Journal of Medicine (NEJM) with 2,246 patients found that treatment of symptomatic Covid-19 with Nirmatrelvir plus Ritonavir resulted in an 89% lower risk of progression to severe Covid-19 when compared to placebo. This study was sponsored by the manufacturer, Pfizer. Safety: Looking at the overall data, Paxlovid does not appear to have any significant safety concerns.   • What Is The Best Vegan Certification?

    The issue with vegan certifications lies in the fact that they contradict the central ideology of veganism. The definition of veganism, as described by PETA is, “A vegan (strict vegetarian) does not consume meat, dairy products, eggs, honey, or any product derived from an animal. A vegan diet can (and should) be full of a wide variety of delicious, nutritious foods, including vegetables, grains, nuts, legumes, seeds, and fruits. Vegans don’t wear leather, fur, silk, or wool. Many refuse to use products that are made with animal ingredients, products that are filtered using animal parts (such as some wines, beers, and white sugars), and products that have been tested on animals. ” Similarly, The Vegan Society’s definition is, "A philosophy and way of living which seeks to exclude-as far as is possible and practicable-all forms of exploitation of, and cruelty to , animals for food, clothing or any other purpose; and by extension, promotes the development and use of animal-free alternatives for the benefit of animals, humans and the environment. In dietary terms it denotes the practice of dispensing with all products derived wholly or partly from animals." Vitamins, over-the-counter (OTC) medications, and prescription drugs have a difficult time abiding by these definitions of being “vegan” because animal testing is usually a prerequisite for the development of these products. For this reason, many companies avoid the terminology “vegan” as it could be considered inaccurate and it places the manufacturer at risk of undue misbranding and/or mislabeling. For example, the heme used by Impossible Foods was tested on animals and hence vegan certification would not be appropriate. The calcium in a multivitamin has been tested on animals previously and the multivitamin is technically not vegan. A more accurate and preferred certification would be one using the terminology, “animal-free” (e.g. Certified Animal-Free). Certified Animal-Free aligns with the following: The American Medical Association (AMA) uses this terminology The AMA urges manufacturers to include all ingredients and components present in medical products on the product label, including both active and inactive ingredients, and denote any derived from an animal source. They also encourage cultural awareness regarding patient preferences associated with medical products containing active or inactive ingredients or components derived from animal sources. Medical Information departments typically do not use the word “vegan” Most medical information departments will provide their responses as “Our product does or does not contain animal-derived ingredients”. This is in part due to the inaccuracy of calling the product vegan for the reasons described above. Is inclusive and incorporates all ideologies Globally, millions of individuals avoid animal-derived ingredients for a multitude of reasons, including health, animal cruelty, dietary preference, environment, and religious or personal beliefs.   • Clearing Your Hearing With Animal-Free Debrox Products

    Most people may not notice or think about what is inside their ear canals until there is a problem. Whether their ears are clogged with too much earwax or flooded with water, the pain and discomfort can make anyone scramble for a quick solution. For a lot of people, that solution is Debrox, a brand recommended by doctors and pharmacists alike. What is earwax? One of the most common causes of clogged ears is excess cerumen. Cerumen is another word for earwax, which is a slurry of mainly sloughed-off skin cells and oils. Although it does not sound very appealing, earwax does actually help protect the ears. While inside the ear, it is moist enough to act as a lubricant for dryness and sticky enough to trap dirt, debris, and even small insects! It also can contain antimicrobial proteins secreted by your cells to ward off bacterial growth. Normally, earwax steadily churns its way out of the ear while you talk or chew food since the movement of the jaw helps push it along. As the earwax moves, it dries out, flakes off, and takes away anything it has trapped on the way out. However, if earwax builds up faster than it gets churned out, muffled hearing, and discomfort can occur. The simplest solution for this problem is to remove it. Q-tips and cotton swabs can push earwax further inside the ear canal, making it harder to remove and/or possibly damaging the eardrum. Ear candles are marketed as tools for wicking earwax towards the flame. However, like Q-Tips and cotton swabs, candles do not safely and effectively remove earwax. Candles for earwax increase the risk of burns due to close proximity with an open flame and hot ash falling into the ear. How can I remove earwax? The safest over-the-counter option to rid the ear of excess earwax comes in the form of Debrox Earwax Removal Drops . This product contains the active ingredient carbamide peroxide 6.5%. When introduced into the ear, the mixture begins to fiz and foam, releasing tiny bubbles of oxygen, softening and loosening earwax. Carbamide peroxide also breaks down the keratin chains with its keratolytic properties, making it easier for the foaming bubbles to dislodge stuck earwax. Softened and loosened earwax can be removed passively or by flushing the ear with warm water and a soft rubber bulb syringe. The adhesion and surface tension of water can make it hard to drain from the ear. Persistent moisture in the ear can create an ideal breeding ground for unwanted bacteria. Swimmer’s ear is an informal name for “otitis externa”, an infection of the outer ear canal. Symptoms may include ear itching, redness, inflammation, pain, discomfort, and muffled hearing. How can I remove water in my ears? Debrox also has a product called Debrox Swimmer’s Ear Drying Drops. These drops are a mixture of isopropyl alcohol 95% stabilized in anhydrous glycerin 5%. When these drops enter the ear and mix with the water present, it allows for rapid moisture evaporation and bacterial growth prevention. Using these drops can help prevent a Swimmer’s Ear infection from occurring. Glycerin is an ingredient that can be sourced from both animal and plant-derived sources. The glycerin and other ingredients in Debrox Swimmer’s Ear Drops have been verified to not be sourced from animal-based sources. Typical directions for use are: Place 4 to 5 drops into the affected ear(s) Listen to your hearing If your ear problem is not due to earwax or water, these products may not be suitable for you and may require the help of your doctor or other medical professionals. ALA is commonly found in vegetable oils, walnuts, flaxseed oil, leafy vegetables, and grass-fed animals. EPA and DHA are commonly sourced from fish, seaweed, and algae. Interestingly, although fish are a great source of EPA and DHA, they too cannot synthesize these omega-3 fatty acids and obtain them through their diet of microalgae and plankton. The human liver converts about 15 % of the ALA we consume into EPA and DHA, which highlights the importance of consuming these essential fatty acids through our diet and supplementation. Recommended Omega-3 Intake Per the National Academy of Medicine, adequate intake of ALA is 1.1 g in healthy adult females and 1.6 g in healthy adult males. According to the United States Department of Agriculture’s Dietary Guidelines for Americans 2015-2020 and the World Health Organization, the average consumption of 250 mg per day of EPA and DHA meets the dietary recommendation for consuming 8 ounces of seafood per week. The American Heart Association recommends a combination of the above by encouraging at least 2 servings of fish per week and plant-derived omega-3 fatty acids, but for a recommended range of 1.5 - 3 g of ALA or a combined daily dose of 0.5 - 1.8 g of EPA and DHA. An adequate intake of ALA can easily be achieved with vegan and vegetarian diets as ALA is mostly found in plant foods. As DHA and EPA are more commonly found in fish, algae-based DHA and EPA are animal-free alternatives suitable for vegan and vegetarian diets. Omega-3 Studies and Results on Cardiovascular Health Several studies have found a correlation between the adequate intake of omega-3 fatty acids and the reduced risk of coronary heart diseases. Most of the present data is centered around the consumption of EPA and DHA from seafood and not ALA which is highly found in plant-based foods. Two recent studies, VITamin D and OmegA-3 TriaL (VITAL) and A Study of Cardiovascular Events in Diabetes (ASCEND), compared the effects of omega-3 fatty acids against placebo in primary prevention of cardiovascular disease (CVD). In both studies, participants were given a daily 1 g omega-3 formulation composed of 460 mg of EPA and 380 mg of DHA. VITAL included 25,871 patients aged 50 and older with no previous history of heart attacks, strokes, or cancer. The ASCEND trial included 15,480 adults aged 40 and older with diabetes but no history of CVD. Participants in both studies showed beneficial heart-healthy outcomes such as reduction in heart attack, coronary heart disease, and percutaneous coronary intervention. The Reduction of Cardiovascular Events with Icosapent Ethyl-Intervention Trial (REDUCE-IT) studied the consumption of high dose EPA (4 g/day) in middle to older aged individuals who had elevated triglyceride levels, a history of cardiovascular (CV) events, or an increased risk of a CV event. The study concluded that the intake of high dose EPA in patients with elevated triglycerides on statin therapy led to a 25 % reduction in CV events when compared to placebo. Conclusion There is a variety of omega-3 fatty acids available over-the-counter (OTC) from both plant-based and animal-based sources. The recommended dose of omega-3 fatty acids differs based on the needs and health conditions of the consumer. Clinical trials on the benefits associated with the consumption of omega-3 fatty acids support manufacturers’ health claims which indicate improved cardiovascular health. Conflicting data exist on the benefits associated with eye health, neurodevelopment, and cognitive function.
