  • Your Essential Guide to COVID-19

    The coronavirus pandemic has upended our day-to-day lives and saturated the news and social media feeds. With so much information (and non-information) coming at us, it's hard to filter through to what is actually important. Our team is going through and doing the work for you. Keep checking back for updates! April 16, 2020 Status Report According to the CDC , there have been 632,548 cases and 31,071 deaths in the US. A study conducted of COVID-19 patients in an academic hospital in New York City suggested that obesity increases the need for hospitalization and critical care. The study surveyed 3,615 patients and found that 21% (775) had a body mass index (BMI) 30-34 and 16% (595) had a BMI above 35. An individual is considered obese with a BMI greater than or equal to 30. The researchers concluded that "though patients aged <60 years are generally considered a lower risk group of Covid-19 disease severity, based on data from our institution, obesity appears to be a previously unrecognized risk factor for hospital admission and need for critical care." Given that nearly 40% of adults in the United States are obese, this study could have serious implications. The question on everyone's mind is when will a COVID-19 vaccine be ready for distribution? At the beginning of April, Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, said that it would take 12 to 18 months for a vaccine to develop, test, and then be approved for public distribution. On April 4, WHO reported that 60 candidate vaccines are currently in development worldwide and two have entered the first phase of clinical evaluation. Top 3 things to do... STAY INSIDE - maintain social distancing (or physical distancing) by staying at home as much as possible. Only go out for essentials (groceries and toiletries). Feel free to go to parks/trails for hikes, but maintain at least 6 feet of distance from others. If you do go to the store, only send one person, go with a list, and be mindful of those around you. WASH YOUR HANDS - Regularly wash your hands with soap and water. If soap and water is not available, use hand sanitizer with at least 65% alcohol content. Wash your hands after every outing. CLEAN SURFACES - Regularly wipe down doorknobs, phones, countertops, light switches, and other frequently used items. Is there a tea to cure COVID-19? There has been a viral post making its way around social media claiming that drinking tea will cure COVID-19 (see below). There is no evidence suggesting that tea could help you fend off the coronavirus. " There is also no evidence that Dr Li Wenliang was researching the effect of methylxanthines, and the fact that he was an eye specialist and not a virus specialist makes that particular claim unlikely." It is correct that methylxanthines are found in tea and other food and drink, such as chocolate and coffee. Theophylline and dyphylline can also be used to treat respiratory illnesses like asthma and bronchitis, but require much larger quantities than is found in tea. As of today, the World Health Organization says that there is no medicine that can prevent or treat COVID-19. Should I wear a mask or respirator? Which one? Good news! If you're able to shelter at home, you don't need a mask at all! Here are the different options that you will hear about, and when they are appropriate... 1. Surgical face mask Face masks are effective as preventing the spread of contagions FROM you, but they are not very effective at blocking contagions in the air. What does that mean? If you are symptomatic, then wearing a mask will help decrease the spread to others (but self-quarantine is the BEST way to prevent spread). UPDATE 3/31/20: If you are not symptomatic, you may consider wearing a mask in public. The CDC is re-evaluating its previous stance on reserving masks for the sick and those caring for the sick because of the high rate of asymptomatic COVID-19 carriers. We again encourage you to reserve masks for healthcare workers, but wearing a scarf or cloth mask will still help prevent the spread of respiratory droplets while speaking. 2. N-95 respirators Respirators will protect you from inhaling the virus by filtering the air before it is inhaled. These masks come in sizes and must be fit for each individual to ensure efficacy. Right now, all of these respirators should be reserved for our healthcare providers on the front lines. If you have any N-95 respirators, please consider donating them to your local hospital, doctor's office, or pharmacy. Fortunately, Duke University researchers recently found that respirators can be sterilized and re-used using vaporized hydrogen peroxide. See more of their research HERE . 3. Cloth masks I'm sure you've seen a lot of people making face masks for our healthcare workers, and that's incredible. Unfortunately, the research we have so far shows that cloth masks are very ineffective at preventing inhalation of the virus ( MacIntyre CR, Seale H, Dung TC, et al. BMJ Open . 2015;5(4) . ) While cloth masks are better than nothing, this research reminds us that it's even more important for respirators to be reserved for hospitals! Are there any medications that will work? Possible therapies have been tested all over the world, and some have shown some minor efficacy. The American Society of Health-System Pharmacists is an excellent resource for more technical information. Their COVID-19 dashboard can be found HERE and is the source for the summaries below. A quick note on all of these medications: the current supply chain is limited. Treatment should be reserved for the sickest patients - those admitted to the hospital. Please, please do not try to acquire prescriptions for these medications "just in case." Hydroxychloroquine/Chloroquine Why are we using it? Both of these medications have in vitro (in a test tube) activity against the virus and effects on the immune system that may be beneficial during infection Does it work? So far, there is limited data to determine whether there is a benefit, however, several case reports suggest that there may be benefit. Aquarium products contain chloroquine...can I just take that? Please don't! These products also contain harmful chemicals! Recently, a man died and his wife is in critical care after taking these products as a "preventive treatment." Lopinavir/Ritonavir (Kaletra) Why are we using it? This medication is an antiretroviral with in vitro (in a test tube) activity against SARS-CoV and MERS-CoV (other novel coronaviruses). Does it work? A small, open-label trial did not show benefit from using Kaletra and had to be stopped early in several patients due to adverse effects. Remdesivir Why are we using it? Remdesivir is a novel, broad-spectrum antiviral not currently commercially available. Does it work? Three trials are on-going, but this is the most promising treatment for COVID-19 at this time. Vitamin C Why are we using it? Vitamin C is an antioxidant and may support host defenses during infection. Does it work? IV Vitamin C is currently used for patients with sepsis and shows benefit. LIttle clinical evidence is available for use in patients with COVID-19, but one trial with ICU patients is ongoing. Azithromycin Why are we using it? Isn't is an antibiotic? Yes, azithromycin is an antibiotic, but it has some antiviral activity. Additionally, it has anti-inflammatory properties that may be beneficial. Does it work? There is very little evidence to show that azithromycin will be beneficial for patients with COVID-19. Studies with MER-CoV did not show benefit. Additionally, both azithromycin and hydroxychloroquine prolong the QT interval, which may lead to life-threatening arrhythmias.   • FDA Citizen Petition: Demanding Change

    On July 10, 2023, our team filed a Citizen Petition to the FDA urging them to take vital action on the labeling of animal-derived ingredients in health products. This petition stems from concerns regarding the current lack of transparency and the challenges faced by many consumers with health-related, dietary, religious, or moral restrictions. Many individuals, including those with Alpha-gal Syndrome (AGS), allergies, religious dietary requirements, and ethical concerns, are unable to effectively identify and avoid products containing animal-derived ingredients. What is a Citizen Petition? A Citizen Petition is a formal request by individuals or groups to the government, urging specific action on an issue. It allows citizens to voice concerns and influence public policy for positive change. A Citizen Petition differs from a general grassroots petition because, under federal agency rules, the agency must respond to a Citizen Petition. How does a Citizen Petition work? What changes are we requesting? Require drug and dietary supplement manufacturers to clearly label whether ingredients used in their products are animal-derived. Encourage manufacturers to replace animal-derived ingredients with plant-based and/or synthetic alternatives when viable options are available. Confirm that labeling of drugs and dietary supplements may include a certification symbol denoting the absence of animal-derived ingredients. Why is this Citizen Petition important? Currently, drug and dietary supplement manufacturers are not required to disclose whether their products contain animal-derived ingredients. This lack of labeling transparency poses challenges for consumers and healthcare professionals alike.     In 2022, Katrina Hagan transformed her personal journey with lupus and food sensitivities into a mission to bring joy back to the table for those with dietary restrictions. What began as home-baked treats evolved into Sensitively Sweet , a Nixa, Missouri bakery dedicated to creating delicious, allergen-free delights. In this interview, Katrina shares the story behind the bakery's name, her commitment to ingredient transparency, and how community support has been integral to her success . Tell us your story of how Sensitively Sweet was born. Who created the name?  Sensitively Sweet was born out of necessity-and love. When I was diagnosed with lupus and developed severe food sensitivities, and my husband was diagnosed with Alpha-gal Syndrome , finding safe treats became almost impossible. We missed the joy of sharing delicious desserts with others , and I knew we weren't alone. So I started baking allergen-free sweets in my kitchen , carefully crafting recipes that were free of the top allergens and Alpha-gal safe. The name "Sensitively Sweet" came from a friend who suggested it after hearing how many different food sensitivities I was baking for it just fit perfectly.  Can you share a bit about what you do and your personal stories with Alpha-gal and other allergies?  Living with a spouse who has Alpha-gal Syndrome and dealing with my own autoimmune and allergy issues has completely changed how I approach food and life. I know what it's like to feel anxious every time you eat something new , or to grieve the loss of your "old" diet. That experience pushed me to learn everything I could about hidden allergens, cross-contamination, and ingredient transparency. Learn to read labels carefully, find your safe staples, and don't be afraid to ask questions. Connect with others in the allergy community-you are not alone. And most importantly, give yourself grace.   • Living with Alpha-gal Syndrome: Cortney McCord Inspires Change and Hope

    Meet Cortney McCord, a passionate advocate and warrior navigating the challenges of Alpha-gal Syndrome (AGS) . Diagnosed in 2021, Cortney has faced the hurdles of delayed diagnosis, managing symptoms, and reshaping her daily life to avoid triggering reactions. Determined to make a difference, she’s become a driving force in the AGS community—leading Facebook groups, sharing vital information, and championing legislative change to support others living with AGS. Can you tell us a little bit about yourself? I am a registered nurse. I volunteer my time as a nurse educator in the Facebook support groups that I started. I am currently a full time college student pursuing my masters degree in dietetics and nutrition with the goal of becoming a registered dietitian.  When were you diagnosed with AGS? Did you know about it previously? My first anaphylaxis happened in July 2021. My very first reaction happened when I fried pork sausage for my family for breakfast. I had anaphylaxis from inhaling the airborne alpha-gal from the sausage. I did not know a lot about alpha gal at that time, but I was aware of it thanks to contracting Lyme and other tickborne diseases in May 2021. I was really sick at the beginning. I was not believed by the first allergist I saw. He thought it was impossible to react simply to airborne alpha-gal without eating the meat. It took me 8 months to receive adequate care from an allergist. I had to travel to see Dr. Scott Commins , who is the world’s leading researcher for AGS. His excellent care is why I am alive and kicking today! How have you gotten involved in the AGS community? As time passed and I learned more about AGS, I started helping others in the (Facebook) support groups by commenting on others' posts. Then, I saw a need to create my first group–Alpha-gal on a Dime. Then, after some time and with some encouragement from other AGS advocates, I started a group for airborne patients like me– Airborne Alpha-gal (aka fume) Reaction Support Group. Then I started groups for art (Alpha-gal Artists), meal planning (Alpha-gal Meal Planning Hub), Xolair (Alpha-gal and Xolair (omalizumab) Discussion Group), and a local group called Alpha-gal Allies of Northeast Mississippi.  What has been your biggest challenge navigating an AGS diagnosis? The biggest challenge for me in navigating my AGS diagnosis has been the lack of education amongst medical professionals. There is a need for more peer reviewed medical literature that teaches practitioners in every specialty how to care for AGS patients. I hope to change that soon for the field of dietetics and nutrition. What motivates you to do the work you do? I started this work when I was so sensitive to airborne alpha-gal that I’d go into anaphylaxis any time I left home. I was having to use epi pens so often that it wasn’t funny. I was so scared I would die from this. My entire family was scared too. I decided that if alpha-gal was going to be the defining feature of my life, that I would make it positive by helping others. I wanted to leave a legacy of service as an example for my family. I say all of this to encourage folks to get involved as they’re able and to help them know that there is hope and treatment even for someone who has been as sensitive as I have been in the past.  What advice would you give to those newly diagnosed with AGS? I would encourage everyone to do what they can to help others. It is a joy. You don’t need any special skills to listen and encourage others. Another easy way to get involved is to reach out to your Federal Representatives. Ask them to cosponsor HR 9382– the Alpha-gal Allergen Inclusion Act.  This legislation will declare alpha-gal the tenth major food allergen . If passed, food labeling will become mandatory. All I had to do was ask my Congressman’s health aide if he would co-sponsor the bill and he did! It only took a phone call and an email. I’d encourage everyone to do this for themselves and our community. Another tip I would have for those who are new to AGS is to start with this Checklist for the Newly Diagnosed  from alphagalinformation.org . It includes information on how to recognize and treat anaphylaxis, which is so important. How has Pill Clarity impacted your experience with AGS? I really needed a service like Pill Clarity from the very beginning. My first FB post in an alpha-gal group was asking if the rescue meds in the ER would be safe for me. I had been taking Armour Thyroid , which is made from pork thyroid glands. It sent me into the worst anaphylaxis I’ve ever had. It was terrifying knowing that a medication made me this ill and not knowing if the medications I’d be given would help me or harm me further. Thank you for your hard work!   • Hidden Risk in Stelara and Cetuximab for Alpha-gal Syndrome

    Biologics are transformative treatments for autoimmune diseases, chronic inflammation, and certain cancers. However, some biologics carry hidden triggers that can cause severe allergic reactions in certain patients. One such trigger is alpha-gal (galactose-α-1,3-galactose). Notable examples are Stelara (ustekinumab) and Cetuximab—monoclonal antibodies produced using mouse cell lines. These drugs can carry alpha-gal on their surfaces, posing a risk for those with Alpha-gal Syndrome . The Alpha-gal Connection Stelara and Cetuximab are produced in cell cultures that can incorporate alpha-gal into their molecular structure. Stelara , used for inflammatory bowel diseases such as Crohn’s and ulcerative colitis, and Cetuximab , used in treating certain colorectal and head-and-neck cancers, are both generated from the murine Sp2/0 cell line—an environment where alpha-gal can “decorate” the protein. When introduced into a patient with Alpha-gal Syndrome (AGS), these antibodies can trigger the release of histamines and other inflammatory chemicals, resulting in an immediate infusion reaction, even at the very first dose. Current Research  Researchers have looked into patient cases to understand this phenomenon better. Cetuximab has a well-documented history of causing severe first-infusion reactions in alpha-gal patients, especially in certain U.S. regions. Meanwhile, Stelara reactions , although less potent, have also been observed. Interestingly, when Stelara is given subcutaneously (under the skin) rather than intravenously, these reactions appear milder and often manageable, suggesting that the route and concentration might influence the body’s response. Can you share a bit about what you do? I am an independent retail pharmacy owner, so I do everything LOL! I compound medications, counsel women in women’s health surrounding menopause and hormone imbalances, immunize patients, and provide consultation to patients with AGS. Tell us about a formative moment in your life that makes you the person that you are today. When I was thirteen years old my grandfather passed into Heaven and before he left me he said something that has stuck with me. “Get all the knowledge about everything you can. Knowledge is the one thing no one can ever take from you.” So, I consider myself a perpetual student. I also love to teach which fits right in with constantly learning. I love to teach people the “why”. I believe that teaching the why behind something helps people be more compliant which produces healthier outcomes. What motivates you on a daily basis? I have a servant's heart and I love people. I want to help people heal and prevent illnesses as any good clinical pharmacist should! But I truly strive to treat everyone how I would treat my own grandmother and how I would want them to treat my son or grandchildren. I want to teach people how to overcome the medical barriers of life. I am not a diagnosing physician nor am I a dietician, so it is so important for us all to work together in the healthcare community! Can you tell us a personal experience that pulled you into the animal–derived ingredients and alpha-gal landscape? I have a niece and several patients who couldn’t eat at family gatherings without having special foods prepared for them. The more aware I became of the challenges they faced, I started taking notice that the medications were making them feel worse so since I specialize in medications, this is where I can help! I began digging deeper, working with each patient individually to break down their issues and find better solutions. How long have you known about AGS? How did you discover it? I have been in pharmacy for over 25 years and have been aware of various tick-borne illnesses since I was a little girl as I suffered from RMSF (Rocky Mountain Spotted tick Fever). However, AGS was brought to the forefront of science literature in 2009, with the first case being diagnosed in Oklahoma around 2020. I was employed by IHS as an urgent care pharmacist and there was an individual who started presenting with strange allergic reactions including anaphylaxis. Northeastern Oklahoma, particularly where we live in Tahlequah, is heavily wooded and known for its outdoor activities such as abundant hiking trails, agricultural land, and the Illinois River. What was the most surprising thing you discovered about AGS? No two cases are the same and there are so many people being newly diagnosed who have been suffering for years! I seldom talk to someone who shares their story with me that doesn’t become emotional because they finally have an answer to the history of problems taking medications and random allergies. Most of the time they were dismissed by their providers because there just wasn’t an awareness like there is now. They are so relieved and thankful that you are there to walk through it with them and they are not alone. What do you think is the most challenging aspect of navigating an alpha-gal diagnosis with a patient? The fact that every diagnosis is so unique with the varying degrees of sensitivities. We have a few patients that can not walk into our pharmacy during lunch because of the meat from the barbecue being cooked in the building. Then we have other patients that simply cannot eat meat without nausea. So I think the biggest challenge is trying to find the patient's level of allergy and what they think they could be sensitive to without knowing. What was the most challenging part of learning where to look for animal-free medications? Time! I would—and still do—ask my patients to bring me a list of the over-the-counter and prescription medications they’re taking so I can start to find alternatives for each one. It was very time consuming because I would call and or email the manufacturers for each drug the patients were taking to verify the medication was free from mammal ingredients. Then I would look at any other specific allergies the patient was experiencing with any of the remaining fillers, binders, or tablet coatings. What changes did Tahlequah Drug Co. implement to accommodate AGS patients? We have a specific over the counter medication section that is mammal-free. We also have a procedure for AGS patients. When the pharmacist checks medications for an alpha-gal patient it must have the yellow “alpha-gal friendly” sticker on it to verify the medication is alpha-gal friendly. Also, there is a flag note that appears on the patient's profile every time a prescription is sent in to alert the technician typing and again when the drug is checked at the pharmacist final check. What advice would you offer to other pharmacists so they could better equip themselves to manage AGS? Listen to your customers and build a relationship with them. Join a local AGS group through once monthly meetings or social media platforms. Read the journals and listen to the credited podcasts. I have gleaned a lot of helpful information from the PCCA podcast. Team up with your local allergy clinic. Let them know you are there to assist and be willing to LISTEN to what the patient’s individual needs are, take notes, and make notes in the computer on their profile that will help you make relevant decisions for their medications based on their individual needs. Is there anything else you would like to share with us?     Cora is the newest member of our team and is joining us as our Medical Information Fellow! Cora, a recent graduate from the University of Connecticut, moved across the country to pursue a career deeply rooted in a cause she passionately believes in. Cora believes that everyone should have access to safe medication and wants to be part of the solution in increasing both medication transparency and availability of animal-free medications. Why did you join the Pill Clarity team? I was first introduced to Pill Clarity through an APPE (Advanced Pharmacy Practice Experience) rotation that my pharmacy school offered. Throughout this rotation, I was continuously surprised by how big the need for animal-free medications was, especially considering that this topic was never broached in any of my schooling. I thought that what the Pill Clarity team did was absolutely amazing and I kept in touch after my rotation. When the team mentioned that they were looking for an intern, I jumped at the chance. I got to work on projects that I wasn't able to touch within a short 4 week rotation and help people that didn't have anywhere to turn to for information regarding their medication. When a fellowship position opened up, I decided I wanted to continue to help a population that was often overlooked (e.g. those with Alpha-gal Syndrome and other allergies). I feel that I'm really able to make a difference for the people who send in questions and at the end of the day that's what makes all the trials I've faced worth it. What was the most shocking thing you discovered about the pharmaceutical industry when you started working here? The most surprising thing I've encountered is the lengthy response times from some pharmaceutical companies. I'm receiving replies now for inquiries I sent over six months ago at the start of my internship. I've also heard about emails arriving for students who assisted us and have since graduated. This turnaround time, especially concerning medications that people rely on for survival, is quite baffling. This highlights the importance of the work Pill Clarity is doing. I can't imagine patients having to juggle multiple medications and contact various manufacturers daily, especially when it takes an average of 13 minutes to get an answer from just one manufacturer. What was your favorite Pill Clarity project or initiative you've worked on so far? My favorite Pill Clarity initiative I work on would have to be med info. Being able to help patients find suitable medications for their health needs is empowering and hearing their stories is motivating.   • Medication Insights from Alpha-gal Social Media Groups

    Alpha-gal syndrome (AGS) is becoming more common, affecting over 450,000 people in the US. It's an allergy triggered by a sugar molecule called α-gal, found in most mammals which can be transmitted from the Lone Star Tick. This allergy is often ignored and makes taking medications tricky because 75% of them have ingredients from animals. What's worse, 75% of AGS patients react to these ingredients, with 50% having severe reactions. A study presented at the American College of Cardiology 73rd Annual Scientific Session & Expo looked at discussions in social media groups for AGS patients about medication. The goal was to understand how hard it is for these patients to deal with medication. Unraveling the Medication Conundrum Patients with AGS are not only vigilant about their dietary choices but also meticulously scrutinize their medication intake. The study revealed that discussions about medications (20%) in AGS social media groups were nearly as prevalent as those about food (22%). Given the prevalence of animal-derived ingredients in medications, transparent labeling is crucial for patients managing AGS. Insights from the Study Researchers looked at posts from three AGS support groups on Facebook, which represented 5% of all members. They analyzed 1,195 posts and sorted them into different categories like treatments, symptoms, and medications. The frequency of drugs mentioned within these groups revealed some noteworthy findings. Diphenhydramine , cetirizine , fexofenadine , and ibuprofen were among the most discussed medications, highlighting the significance of antihistamines and pain relievers in managing AGS-related symptoms. However, what's concerning is the presence of animal-derived ingredients in some of these medications, potentially posing risks to AGS patients. A Call for Transparent Labeling The study underscores the high patient burden associated with identifying safe medications for AGS management. Transparent medication labeling , such as indicating certified animal-free products, could significantly alleviate this burden and enhance patient safety.
